Purpose

The World Health Organization has declared the novel Coronavirus (COVID-19) a worldwide pandemic. COVID-19 can be transmitted by asymptomatic people through respiratory droplets, as part of health and safety best practices to endeavor to keep both Members and Associates safe Members will be required to complete a health check each time they visit, and a one time declaration / acknowledgement waiver periodically which is subject to change and guidance from local and Federal Government legislation.

I am not experiencing or exhibiting any of the following COVID-19 like symptoms (Sore throat/ Shortness of Breath/Difficulty Breathing/ Fever/Chills/ Headache/ Sneezing/ Nausea/ Coughing/ Muscle Pain/ Loss of Taste or Smell)

I have not travelled outside of the country without completing the 14-day mandatory quarantine.

I have not tested positive for COVID-19 or had contact with a person confirmed sick with COVID-19 without wearing appropriate PPE in the last 14 days.

Ottawa Public Health has not advised me or anyone in my household to self-isolate due to pending test results in the last 14 days.

I agree to abide by all Rideau Club COVID-19 policies and protocols.

The Rideau Club reserves the right to refuse any Member or guest access to the club if there are any concerns around recent travel, or a Member or guest has had recent contact with a person confirmed sick with COVID-19. Persons with a temperature of 37.8° or greater will be refused admittance.

ACKNOWLEDGEMENT AND DECLARATION

I acknowledge whilst the Rideau Club cannot guarantee that you will not contact or transmit COVID-19, you agree that your visit to the Rideau Club is entirely at your own risk and by doing so release the Rideau Club, its Directors, Officers and Associates from any and all liabilities, demands including any loss, injury or death directly or indirectly arising out of visiting the Rideau Club.

I certify that the above information is true and understand that any misrepresentation made on my Daily Health Declaration or any failure to follow the COVID-19 rules and procedures established by the Rideau Club for the purpose of maintaining the well-being of their associates, members, guests and other visitors may result in my membership coming under review possibly resulting in termination.

This waiver will remain effective until laws and mandates relevant to COVID-19 are lifted and will be updated in accordance with.
